Print this character reference form and give it to a mature Christian friend who you are not related to, and who can best answer questions about your character and Christian lifestyle. Please have them email the completed form to internships@outback.church or include for your character reference a pre-stamped & pre-addressed envelope to mail the form to:

Final step

After all six steps are completed and reviewed by Lakeview SRCM, we will contact you to schedule an admission interview. 

All interns at Lakeview School of Rural Church Ministry are required to pass a background check, to be completed by Lakeview School of Rural Church Ministry upon approval to the program.
